Each year under the Affordable Care Act (ACA), AutoNation is required to send a Form 1095-C regarding your eligibility for health insurance. You can choose to receive this form electronically, instead of via paper form in the mail.

How do I create an account on the AutoNation portal?

  1. Go to eyacaforms.com/AutoNation.
  2. Click on “Register as a new user.”
  3. Enter your email address and a password. We recommend you use a personal email.
  4. To create an account, you will need to enter the last four digits of your Social Security number, your last name, and your date of birth.

How will I receive my Form 1095-C electronically?

  • You must create your account and select the electronic delivery option by December 15.
  • You will receive an email notification when your form is available for download.
  • You can change your election for the current year at any time by logging on to www.eyacaforms.com/AutoNation.
