Under The Paperwork Burden Reduction Act, employers are no longer required to provide the Affordable Care Act (ACA) Form 1095-C regarding your eligibility for health insurance. However, you can obtain this form electronically, or request that a form be sent to you by following the steps below:

How do I quickly obtain my
1095-C form electronically?

  1. Create an account on the AutoNation portal for a new form or reprint of a prior year’s form.
  2. Go to eyacaforms.com/AutoNation.
  3. Click on “Register as a new user.”
  4. Enter your email address and a password. We recommend you use a personal email.
  5. To create an account, you will need to enter the last four digits of your Social Security number, your last name, and your date of birth.

What other ways can I request my Form 1095-C without using the portal?

  • Send an email requesting your 1095-C form to ASKHR@AutoNation.com.  Make sure to include your full name, date of birth, and the form’s year that you are requesting.
  • Send a letter requesting your 1095-C form to AutoNation Benefits, 200 SW 1st AVE, Suite 1400, Fort Lauderdale, FL 33301.  Make sure to include your full name, date of birth, phone number, mailing address and the year of the form(s) that you are requesting.
  • Call 1-877-550-BENE (2363).
